Implementing System Safety Lessons Learned (AIHce EXP 2023 OnDemand)

Course Description:
Recorded at AIHce EXP 2023

A system safety process can improve safety when applied correctly. However, inappropriate use of system safety methods can increase risk. Project examples will be provided to support the discussion of lessons learned for implementing the key steps in the system safety process. Example projects and accidents will illuminate: 1) how hazards can be misidentified, improperly assessed, and ineffectively controlled; and 2) how misapplication of surveillance and testing may provide a false sense of security for controlling risks. The topic should provide a lively discussion and audience participation is expected.

Learning Outcomes:
Upon completion of the session, the participant will be able to:

• Discuss lessons learned from implementing system safety programs.
• Incorporate system safety literature and materials provided by the Chemical Safety Board (CSB) into an organization's program.
