This case study will provide examples of how we developed and implement an enterprise-wide accident/incident and near miss management system at Woods Hole Oceanographic Institution (WHOI) and what we are doing to increase participation. Some redacted examples of accident/incident and near miss investigations at WHOI will be shared.

Universal Passive Sampling for Identification and Quantification of Firefighter VOC Exposure
For decades, Firefighters across the world have suffered from illnesses and cancers related to prolonged, repeated occupational chemical exposures. Utilizing XCel+, a novel, universal passive vapor sampler, the XploSafe team worked alongside Arizona-based ISO Class 1 fire departments to characterize and quantify both known and unknown VOC and carcinogen exposures across a wide range of firefighting scenarios. Evaluating VOC presence in the air, on contaminated gear, and inside of various fire apparatuses both during and after fires, the XploSafe team aimed to help departments optimize the post-fire decontamination process to reduce prolonged exposures to these chemicals, and to ensure long-term firefighter health and safety.
