Challenging Incident Investigation Paradigms Webinar Recording



Course Description:
As workplace safety and accident prevention methods have evolved, numerous paradigms have emerged around incident investigations. For example, ‘each accident must have a root cause analysis, and ‘supervisors are the best persons to lead investigations’. With new research findings related to the causal factors of serious injuries and fatalities, it is time to take another look at incident reporting and investigation processes. In this webinar, we will talk about the limitations of the current paradigms, and the need to shift our thinking around incident investigations.

Prerequisites:

Experience in conducting root cause analysis investigations and knowledge of critical safety systems such as lockout/tag out, confined space entry, work at heights.

Upon completion, participants will be able to:

  • Identify accident investigation paradigms that may exist in your organization.
  • Understand the foundational basis for these paradigms and why they create problems in accident investigation effectiveness.
